We require that all new clients read our Policies and Procedures,
fill out a Medical History Intake and sign an Informed Consent.

1. Read: Shade of Green Healing's Policies and Procedures
2. Print and fill out in detail: Client Medical History Intake Form

• We will review this with you when we arrive. Read carefully and be as detailed as possible.

3. Print and sign: Client Informed Consent

• Adults may print and sign the first page
• If the service is for a minor a guardian must sign the second page
